Facility Use Request Form Instructions


    The School District of Holmen welcomes the community to use its facilities. Policy and facility use forms can be found at the left. 


    • Save the appropriate form for the building/area needed and open the file in MS WORD.
    • Complete all requested personal information and information on the event.
    • EVENT START & END DATE – List the day of the week and the date (i.e. Wed, Sept 3).
    • EVENT START & END TIMES – Indicate times, including a.m. or p.m., if setup time is different, list setup time also.
    • MULTIPLE DATES – Please list ALL dates (i.e. Sept 1, 8, 15; Oct. 6, 13, 20 etc.).
    • Trygve Mathison Fine Arts Center – is located in High School.
    • Don Jacobson Auditorium – is located in the Middle School.
    • Please indicate any setup needs, attaching a separate sheet if you need more room for explanation.
    • Save the completed document for your verification.
    • Email the attachment to Brandy Senger at senbra@holmen.k12.wi.us
    • When your request is approved you will receive a signed copy via mail.

    Facility requests should be turned in by July 1 for the upcoming school year to receive priority consideration.
