Physical Card Form
WIAA required physical examination form. Must be dated April 1, 2025 or more recent for the 2026–27 school year. Only page 4 needs to be submitted to the office.

Non-WIAA Competition Waiver
Required for students competing in non-WIAA sanctioned events. Separate forms for HS and MS.

Activity Registration
Register for Holmen activities through Bound. Create a free Family Account, then register your student for any sport or activity. Registration must be complete before a student may practice or participate. WIAA sports require a current physical on file to complete registration.

These meetings are for High School sports and activities. Attendance is mandatory for new participants. If you attended a previous season's meeting, you only need to attend if noted. Use the optional check-in window to verify your physical on file.
