Certificate of Liability Insurance
When District staff or students participate at non-district sites, hosts may request proof of our liability coverage. This proof is provided as a Certificate of Insurance (COI).
When is a COI needed?
COIs are commonly requested when District groups use off-site facilities, sponsor events at outside venues, or collaborate with third parties who want confirmation of our coverage.
How to request a COI
- Download and complete the electronic Request for Certificate of Liability Insurance form.
- Provide full event details: dates, times, location, host organization, and any required wording (e.g., additional insured).
- Submit the completed form to Business Services.
- Allow three (3) business days from submission to receive the certificate.
Fees & billing
The District’s insurance provider may charge a fee to issue a certificate. If a fee applies, it will be charged to the budget area requesting the COI.
Tips for faster processing
- Confirm the host’s exact certificate requirements (additional insured, waiver of subrogation, etc.).
- Provide the legal name and address of the certificate holder as it should appear on the COI.
- Submit requests at least three business days before you need the certificate.
