How to Get Approved

Step 1 — Administrative Authorization

Complete Alternative Vehicle Driver Form #1 for approval to drive on student‑related trips. A criminal background check and motor vehicle record (MVR) check will be requested by a secure third‑party via email.

Step 2 — Medical Opinion (if applicable)

Employees and those receiving compensation (e.g., mileage) must have a Medical Opinion — Form #2 based on a past physical exam. The District does not pay for physicals.

Step 3 — Personal Vehicle Use (if applicable)

If using your personal vehicle to transport students, complete a Vehicle Inspection — Form #3 and provide Proof of Insurance. Form #3 must be signed by your Director or Principal.

Processing time: Form #1 (background + driving record) may take 7–10 days to process. Please plan ahead before any trip requiring approval or a district vehicle.

Insurance & Legal Notes

SDH employees driving personal vehicles: Except for commuting, if you drive your own vehicle in the course of your employment with the District, you have supplemental coverage under the District’s liability insurance; however, your personal insurance is primary. If you drive a District vehicle, you are covered under the District’s insurance. In short, coverage follows the vehicle.

Parents compensated to transport their own children at the District’s request will be required to sign a contract indicating waiver of liability and proof of insurance.

WI DOT: Pupils of educational institutions may not be transported in any vehicle other than a school bus if ten or more students are being transported. For example, 12‑passenger vans commonly used by church groups are not allowed.

Background & Standards

The State of Wisconsin requires drivers of students to be approved in most circumstances. The approval process includes a criminal background check, a motor vehicle record check, and a medical opinion. The use of private vehicles to transport students is allowed if inspected and covered with qualifying amounts of insurance.

Background check email: Look for a message from “Client Services” with the subject line including School District of Holmen. Complete promptly to avoid delays.
