Academic and Career Planning Services for Students
As required by PI 26.03(1)(b)1 of the Wisconsin Administrative Code, the School District of Holmen will inform parents each school year about academic and career planning services their child receives, provide parents with multiple opportunities during each school year to participate in their child’s academic and career planning, and update parents throughout the school year on the progress of their child’s planning. Academic and career planning services are required to be provided to students enrolled in grades 6 to 12. This information can be found on the Instructional Services website.

Asbestos Management Plan
In compliance with the U.S. Environmental Protection Agency (EPA) and the Asbestos Hazard Emergency Response Act (AHERA), the School District of Holmen has an Asbestos Management Plan at the TMT office located at 500 State Street, Holmen, WI. The plan is available for inspection by the public. The District performs six-month periodic surveillance of asbestos in April and October and full re-inspections every three years. Bullying
The School District of Holmen strives to provide a safe, secure, and respectful learning environment for all students in school buildings, school grounds, on school buses, and at school-sponsored events. Bullying has harmful social, physical, psychological and academic impacts on bullies, victims and bystanders. The District consistently and vigorously addresses bullying so that there is no disruption to the learning environment and learning process.
It is the responsibility of school employees, students and parents to promote a bully-free school environment. It is the responsibility of the District to provide support and instruction to reduce and eliminate bullying in all forms.
Students engaging in any form of bullying are subject to school disciplinary actions up to, and including suspension and expulsion from school. Employees engaging in bullying behavior or who witness bullying behavior and do not report it to the building principal or their designee are subject to disciplinary measures as outlined in the Employee Handbook and/or board policy up to, and including, termination. The District highly encourages its staff and students to report bullying via the STOPit Reporting Portal available on the homepage of the District website. Child Nutrition Programs and Free and Reduced-Price Meal Information
Students in the School District of Holmen have an opportunity to participate in school breakfast and lunch. These programs are administered by the District Administrator, in accordance with established guidelines. Menus are posted on the District website. As noted in policy 8500 Food Services, food service prices are established by the Board annually. In accordance with federal guidelines and policy 8531 Free and Reduced-Price Meals, the District will annually notify parents/guardians about the child nutrition programs offered and eligibility requirements for free/reduced meals. The announcement is also shared via local media for the general public. An application is required to verify eligibility, which will remain confidential. The application can be found on the Nutrition Services website, and is available at each school building.
Federal law and USDA regulations require school food authorities to make reasonable modifications to accommodate children with disabilities when the disability restricts the child’s diet. The procedure for requesting meal modifications to accommodate a child’s disability can be found on the the website linked above. Early College Credit Program
According to policy 2271 Early College Credit Program, any 9th through 12th grade student in the School District of Holmen who meets certain requirements is eligible to take one or more courses at an “institution of higher education” for high school and/or college credit. An “institution of higher education” means an institution within the University of Wisconsin system, tribally controlled college or private, nonprofit institution of higher education in Wisconsin. Technical colleges are not eligible institutions under the Early College Credit Program, but are eligible under the Start College Now Program. Refer to policy 2271.01 Start College Now Program for more information regarding options through Wisconsin technical colleges.

Education for Employment
According to PI 26.04(4) of the Wisconsin Administrative Code, the School District of Holmen is required annually to notify parents of its employment program. It must also inform parents of the information and opportunities available to students under the program, including career awareness (elementary level), career exploration (middle school level), career planning and preparation (high school level), academic and career planning services for students grades 6 to 12, and the availability of programs at technical schools. Education of Children and Youths Experiencing Homelessness
The McKinney-Vento Act defines homeless children and youth as:
Children and youth who lack a fixed, regular, and adequate nighttime residence, and includes children and youth who are:
- sharing the housing of other persons due to loss of housing, economic hardship, or a similar reason;
- living in motels, hotels, trailer parks, or camping grounds due to the lack of alternative adequate accommodations;
- living in emergency or transitional shelters;
- abandoned in hospitals;
- living in a primary nighttime residence that is a public or private place not designed for or ordinarily used as a regular sleeping accommodation for human beings;
- living in cars, parks, public spaces, abandoned buildings, substandard housing, bus or train stations, or similar settings; and,
- migratory children who qualify as homeless because the children are living in circumstances described above.
If you are personally aware of or are acquainted with any children or youth who may qualify according to the above criteria, the School District of Holmen provides the following assurances to parents and guardians of children and youth experiencing homelessness and unaccompanied homeless youth:
- The child or youth shall be immediately enrolled and allowed to fully participate in school, even if unable to produce records normally required for enrollment (e.g., academic records, immunization and other required health records, proof of residency, or other documentation) or has missed application or enrollment deadlines during any period of homelessness.
- Children and youths experiencing homelessness are not stigmatized or segregated on the basis of their status as homeless and have full and equal educational and related opportunities.
- Meaningful opportunities to participate in the education of their children including special notices of events, parent-teacher conferences, newsletters, and access to student records.
- Immediate enrollment and transportation to the school of origin. “School of origin” means the school that a child or youth attended when permanently housed or the school in which the child or youth was last enrolled, including a preschool.
- Written explanation of any decisions related to school selection or enrollment made by the school, the local educational agency, or the State educational agency involved, including the rights of the parent, guardian, or unaccompanied youth to appeal and receive prompt resolution of such decisions.

Educational Options
The School District of Holmen Board of Education is required by section 118.57 of the state statutes to annually publish, prior to January 31, a description of the educational options available to the children in the District, including public schools, private schools participating in a parental choice program, charter schools, virtual schools, full-time or part-time open enrollment in a non-resident school district, the youth apprenticeship program under 106.13, and the early college credit program. Human Growth and Development Instruction
As required by section 118.019(3) of the state statutes, on an annual basis, the School District of Holmen will provide parents/guardians with the following information:
- An outline of the human growth and development (HG&D) curriculum used at their child’s grade level,
- Information about how they can inspect the complete curriculum and instructional materials,
- An explanation that no student may be required to take instruction in HG&D generally or in specified subjects within the program if their parent/guardian files a written request that the student be exempted, and
- A statement that students exempted from HG&D instruction will still receive instruction in the subjects under section 118.01(2)(d)2c of the state statutes (knowledge of physiology and hygiene, sanitation, the effects of controlled substances and alcohol upon the human system, symptoms of disease and the proper care of the body), unless exempted, and section 118.01(2)(d)8 of the state statutes (knowledge of effective means by which students may recognize, avoid, prevent and halt physically or psychologically intrusive or abusive situations which may be harmful to students).

Notice of Mental Health Resources
Mental health includes our emotional, psychological, and social well-being. It affects how we think, feel, and act. It also helps determine how we handle stress, relate to others, and make choices. Mental health is important at every stage of life, from childhood and adolescence through adulthood. Over the course of your life, if you experience mental health challenges, your thinking, mood, and behavior could be affected. Many factors contribute to mental health challenges, including:

Family history of mental health challenges
Mental health challenges are common. It is important to normalize conversations surrounding mental health so people can feel empowered to seek the help they need. If you or someone you know is struggling with their mental health, there are ways to get help. Program and Curriculum Modifications
The School District of Holmen is required by section 118.15(1)f of the state statutes to notify students and their parents/guardians of the following at the beginning of each school term:
- their right to request the school board to provide the student with program or curriculum modifications as outlined in section 118.15(1)(d) of state statutes, and
- the decision-making process to be used in responding to such request under sections 118.15(1)(d) and (e) of the state statutes.

School Accountability Report Card (Report Card)
Each public school in the state is required by section 115.385(4) of the state statutes to provide a copy of the school’s accountability report that is published by the Wisconsin Department of Public Instruction to the parent/guardian of each student enrolled in or attending the school. The most recent School District of Holmen and individual school report cards are available on the Instructional Services and DPI websites.
School and School District Performance Report
Annually by January 1, each school board is required by section 115.38 of the state statutes to notify the parent/guardian of each student enrolled in the school district of the right to request a school and school district performance report. The Department of Public Instruction provides a school and school district performance report that includes:
- Indicators of academic achievement, including the performance of students on state-required reading and subject area tests. Testing disruptions and student assessment waivers occurred in the 2019-20 and 2020-21 school year, meaning a 2019-2020 district and school report card were not published. The most recent school accountability report is available on the Instructional Services Website.
- Other indicators of school and school district performance, including dropout, attendance, retention in grade and graduation rates; percentage of habitual truants; percentage of students participating in extracurricular and community activities and advanced placement courses; percentage of graduates enrolled in postsecondary education programs; and percentage of graduates entering the workforce.
- Student suspension/expulsion-related data.
- Staffing and financial data information.
- The number and percentage of resident students attending a course in a nonresident school district, the number of nonresident students attending a course in the school district, and the courses taken by those students.
- The method of reading instruction used in the school district and the textbook series used to teach reading in the school district.
Also, annually by May 1, each school board shall, upon request, distribute to the parent or guardian a copy of the report. DPI does not directly produce discrete, “ready to print” annual school and school district performance reports, therefore the district will provide copies of the 115.38 performance reports to the extent the relevant information is available from the DPI. The data provided in the School District Performance Report is found using the WISEdash Public Portal on the DPI website. Parents and guardians are also able to access and compare other school district’s performance including other school districts in the same athletic conference.
Services for Multilingual Learners (Formerly EL/ESL)
The School District of Holmen is fortunate to have a diverse student population with students who are learning to speak multiple languages. To support all students' access to school the district has dedicated Multilingual Learner (ML) teachers who work with families and students who are learning English. In addition to supporting students, the ML teachers work with families to access resources to communicate with their student’s school and so families can participate in their students' educational experience. Special Education
In recognizing its responsibility to provide children with disabilities appropriate education for all children of the District, the School District of Holmen shall provide free and appropriate programs. Please refer to policy 2460 Programs for Students with Disabilities for detailed information about special education. Special Needs Scholarship Program
This notice serves to inform parents and guardians of students with disabilities that the State of Wisconsin has established the Special Needs Scholarship Program. The Special Needs Scholarship Program (SNSP) allows a student with a disability, who meets certain eligibility requirements, to receive a state-funded scholarship to attend a participating private school. The Special Needs Scholarship Program is governed by Wisconsin Statute 115.7915 and Wisconsin Administrative Code Chapter PI 49.
This is a state-administered program. A parent or guardian who is interested in the Special Needs Scholarship Program should independently verify the participating private schools and the specific terms, eligibility criteria, and application procedures of the scholarship program with the DPI.

Student Assessments
The School District of Holmen Board of Education is required by section 118.30(1m)(d) of the state statutes to annually publish information on the School District of Holmen's Internet site regarding the state-required 4th, 8th, 9th, 10th, and 11th grade examinations administered to student enrolled in the district. Student Attendance
In accordance with WI Statute 118.15, all children between 6 and 18 years of age must attend school full-time until the end of the term, quarter or semester, in which they become 18 years of age, unless special circumstances occur as noted in policy 5200 Attendance. It is the responsibility of any person having under their control such a child to ensure regular attendance during the full period and hours that school is in session, or to provide administration with timely notice and reason for an absence. It is the responsibility of the principal or designee to determine whether the absence is acceptable (excused: i.e., illness, physician’s note, religious holiday) or not acceptable (truant: i.e. skips, illnesses beyond 10 days without medical verification, unexcused tardies). Any student who is identified as a habitual truant will be subject to referral to the appropriate agency in accordance with state law, local ordinances and established procedures. Student Code of Conduct
Students participating in educational programs in classrooms, related school settings, programs, activities, or transportation are expected to demonstrate safe, respectful, and responsible behavior. Any student who does not adhere to the Code of Conduct may be subject to disciplinary action in accordance with established Board policies and school rules. While potential consequences are outlined for various behaviors, all disciplinary decisions are made at the discretion of the building principal or their designee. Each incident is reviewed on a case-by-case basis, considering the circumstances, severity, and student needs, with the goal of promoting accountability and student growth. All actions will be consistent with School District of Holmen policies.
The School District of Holmen is committed to maintaining a positive teaching/learning climate. Teachers are expected to create a positive learning atmosphere for students in their classrooms. Students are expected to behave in the classroom in such a manner that allows teachers to effectively carry out their lesson plans and for other students to learn.

Student Locker Searches
Students have the right to attend school in an environment that is safe, disciplined, and drug-free. To ensure this right, school officials and staff (and law enforcement, when deemed appropriate) need to take steps necessary to maintain the safety and integrity of the school. To this end, searches of students and seizure of their property may be necessary under certain conditions, including while at school-sponsored events or on field trips. Ownership and possessory control of school lockers and desks remains with the School District of Holmen and students/parents have no reasonable expectation of privacy in those lockers and desks. Student/Parent Rights
The Board of Education recognizes that students possess not only the right to an education but the rights of citizenship as well.
In providing students the opportunity for an education to which they are entitled, the District shall attempt to nurture, counsel, and offer custodial care appropriate to their age and maturity. At the same time the Board recognizes that no student may be deprived of the basic right to equal access to the educational program and their constitutional right to due process and free expression and association as appropriate for the school environment.
Parents also have rights in the school system to know about their student’s educational experience. Therefore, parents have the right to inspect any instructional materials used as part of the educational curriculum for their student. Instructional materials means instructional content, regardless of format, that is provided to the student, including printed or representational materials, audio-visual materials, and materials available in electronic or digital formats (such as materials accessible through the Internet). Instructional material does not include academic tests or academic assessments. The procedure used to assure timely response to parental requests to review instructional material can be found in policy 9130.
Student Privacy
The Protection of Pupil Rights Amendment (PPRA) affords parents of elementary and secondary students certain rights regarding the conduct of surveys, collection and use of information for marketing purposes, and certain physical exams. These include, but are not limited to, the right to:
- Consent before students are required to submit to a survey that concerns one or more of the following protected areas (“protected information survey”) if the survey is funded in whole or in part by a program of the U.S. Department of Education (ED):
- Political affiliations or beliefs of the student or student’s parent;
- Mental or psychological problems of the student or student’s family;
- Sex behavior or attitudes;
- Illegal, anti-social, self-incriminating, or demeaning behavior;
- Critical appraisals of others with whom respondents have close family relationships;
- Legally recognized privileged relationships, such as with lawyers, doctors, or ministers;
- Religious practices, affiliations, or beliefs of the student or student’s parent; or
- Income, other than as required by law to determine program eligibility.
- Receive notice and an opportunity to opt a student out of:
- Any other protected information survey, regardless of funding;
- Any non-emergency, invasive physical exam or screening required as a condition of attendance, administered by the school or its agent, and not necessary to protect the immediate health and safety of a student, except for hearing, vision, or scoliosis screenings, or any physical exam or screening permitted or required under State law; and
- Activities involving collection, disclosure, or use of personal information collected from students for marketing or to sell or otherwise distribute the information to others. (This does not apply to the collection, disclosure, or use of personal information collected from students for the exclusive purpose of developing, evaluating, or providing educational products or services for, or to, students or educational institutions.)
- Inspect, upon request and before administration or use:
- Protected information surveys of students and surveys created by a third party;
- Instruments used to collect personal information from students for any of the above marketing, sales, or other distribution purposes; and
- Instructional material used as part of the educational curriculum.
These rights transfer from the parents to a student who is 18 years old or an emancipated minor under State law.
The School District of Holmen has adopted policy 2416 Student Privacy and Parental Access to Information regarding these rights, as well as arrangements to protect student privacy in the administration of protected information surveys and the collection, disclosure, or use of personal information for marketing, sales, or other distribution purposes. The District will directly notify parents of these policies at least annually at the start of each school year and after any substantive changes. The District will also directly notify parents of students who are scheduled to participate in the specific activities or surveys noted below and will provide an opportunity for the parent to opt his or her child out of participation of the specific activity or survey.

Student Records
The federal Family Educational Rights and Privacy Act (FERPA) requires school districts to provide annual notice of student and parent rights regarding student records. As further detailed in section 34 C.F.R. §99.7 of the federal FERPA regulations, parents/guardians and adult students have a right to:
- Inspect and review the student’s education records. All requests for inspection of records should be directed to the building principal.
- Seek amendment of the student’s education records that the parent/guardian or adult student believes to be inaccurate, misleading or otherwise in violation of the student’s privacy rights. All requests for amendment of records should be directed to the building principal.

Consent to disclosures of personally identifiable information contained in the student’s education records, except to the extent that FERPA (and other federal and state laws) authorizes disclosure without consent. School districts are required to make student records available to district employees who are required by the DPI to hold a license, law enforcement officers who are individually designated by the school board and assigned to the school district, and to other school district officials who have been determined by the school board to have legitimate educational interests, including safety interests, in the records.
The district forwards records to other schools that have requested the records and in which the student seeks or intends to enroll, or is already enrolled, as long as the disclosure is for purposes related to the student’s enrollment or transfer. -
File a complaint with the Family Policy Compliance Office of the U.S. Department of Education alleging district noncompliance with FERPA requirements.
The District presumes that the parent has the authority to inspect, review, and obtain copies of records relating to their child unless the District has been advised that the parent does not have the authority under state law governing such matters as guardianship, separation, or divorce. If any education record includes information on more than one child, the parents of those children shall have the right to inspect and review only the information relating to their child or to be informed of that specific information.
Student Records: Directory Data Designations and Opt Outs – Each year, the District shall provide a public notice to students and their parents of the District's intent to make available, upon request, certain information known as 'directory data.' The Board designates as student 'directory data':
- a student's name;
- address;
- telephone number;
- date and place of birth;
- photograph;
- major field of study;
- participation in officially recognized activities and sports;
- height and/or weight, if a member of an athletic team;
- dates of attendance;
- date of graduation;
- degrees and awards received;
- name of the school most recently previously attended.
Parents and eligible students may refuse to allow the District to disclose any or all of such 'directory data' upon written notification to the District within fourteen (14) days after receipt of the District Administrator's annual public notice or enrollment of the student into the District if such enrollment occurs after the annual public notice. Any parent or eligible student who refuses to allow disclosure of directory data and who participates in the co-curricular activity must complete the appropriate acknowledgment, which includes a limitation on the refusal to disclose directory data obtained during the course of the student’s participation in co-curricular activities.

Recruiter Access to Students/Records −In accordance with Federal and State law, the District shall release the names, addresses, District-assigned e-mail addresses (if available), and telephone listings of secondary students to a recruiting officer for any branch of the United States Armed Forces or an institution of higher education who requests such information. A secondary school student or parent of the student may request in writing that the student's name, address, District-assigned e-mail address (if available), and telephone listing not be released without prior consent of the parent(s)/eligible student. The recruiting officer is to sign a form indicating that 'any information received by the recruiting officer shall be used solely for the purpose of informing students about military service and shall not be released to any person other than individuals within the recruiting services of the Armed Forces.' The District Administrator is authorized to charge mailing fees for providing this information to a recruiting officer.
Parents and eligible students may refuse to allow the District to disclose any or all of such 'directory data' to a recruiting officer upon written notification to the District within fourteen (14) days after receipt of the District Administrator's annual public notice or enrollment of the student into the District if such enrollment occurs after the annual public notice. Student Religious Accommodations
The right of the student to be admitted to school and to participate fully in curricular, extracurricular, student services, recreational or other programs or activities shall not be abridged or impaired because of a student's age, race, creed, color, disability, pregnancy, marital or parental status, religion, sex, citizenship, national origin, ancestry, socioeconomic status, sexual orientation, gender identity, gender expression, or gender nonconformity.
The School District of Holmen shall also provide for the reasonable accommodation of a student's sincerely held religious beliefs with regard to examinations and other academic requirements. Requests for accommodations shall be made in writing and approved by the building principal. Accommodations may include, but not necessarily be limited to, exclusion from participation in an activity, alternative assignments, and released time from school to participate in religious activities, and opportunities to make up work missed due to religious observances. Complaints regarding the interpretation or application of policy 2260 Nondiscrimination and Access to Equal Educational Opportunity shall be referred to the Student Services Department and processed in accordance with established procedures.

Title I
Use or Possession of Electronic Communication Devices
According to policy 5136 Personal Communication Devices, the School District of Holmen encourages students to use technology (Internet, mobile device, etc.) in an appropriate manner, for educational purposes, while maintaining the integrity and safety of their learning environment. If a student is observed using a communication device outside the allowable guidelines, disciplinary actions may include confiscation of the device, notification and/or conference with parent/guardian, loss of privilege, detention or suspension, referral to law enforcement if violation involves illegal activity, or any other consequence as deemed appropriate by an administrator.
