Employees and Non-Employees who drive vehicles with students
This information is for those who may be a potential driver on a student-related trip. If you do not anticipate driving students to school-related events you can disregard this information. Please see the Alternative Vehicle Driver Requirements table below.

SDH Employees need to be aware of the following when driving their personal vehicles for transportation:
“Except for commuting, if you drive your own vehicle in the course of your employment with the District, you have supplemental coverage under the District’s liability insurance. However, your personal insurance is primary. If you drive a District vehicle in the course of your employment with the School District of Holmen, you will be covered under the District’s insurance.” In other words, the coverage follows the vehicle.
Parents who are compensated to transport their own children at the District’s request will be required to sign a contract indicating waiver of liability and proof of insurance.
Per WI DOT, pupils of educational institutions may not be transported in any vehicle other than a school bus if ten or more students are being transported. For example, vans commonly utilized by church groups that accommodate 12 passengers are not allowed.
If you want to drive students
Please complete the Alternative Vehicle Driver Form #1 for approval to drive on student-related trips. A criminal background and motor vehicle record check (a request will be sent to you via email by a third-party service) will be reviewed for approval to drive. Once approval has been granted, you will receive a letter stating so.
Employees and those receiving compensation i.e mileage must have a Medical Opinion-Form #2 completed based on a past physical exam. District does not pay for physicals. This is the driver's responsibility.

If you want to use your personal vehicle to transport students
Please complete a Vehicle Inspection-Form #3 along with any additional forms that may pertain to you as stated above. This form must be signed by your Director or Principal in order to be aware of your intent to drive students in your own vehicle.
Please return all completed forms to the TMT Office, (608) 526-4752, Ext. 7200 and indicate on the form to what capacity you are requesting.
Background
The State of Wisconsin requires drivers of students to be approved under most circumstances. The approval process includes a criminal background check, a motor vehicle record check, and a medical opinion. The use of private vehicles to transport students is also allowed if inspected and covered with qualifying amounts of insurance. Click the links below for additional qualification information.

Special Note Regarding Criminal Background Checks: Please be aware that an email will be sent to you from "Client Services" with a Subject Line of School District of Holmen. Please complete the necessary information and submit it as soon as possible in order to expedite your background check.
Please Note
The Alternative Driver Form # 1 which consists of a background and driving record check may take anywhere from 7 - 10 days to be processed and approved. Please allow enough time before any trips requiring a school district vehicle for this process to be completed.
